Comes with one hook.
This hook mounts on inside of cabinet beside your horn speaker. Then you can unhook the chain easily to remove your front grille.
To mount this hook, pre-drill a hole smaller than this hook screw. Then simply twist the screw into your plywood cabinet tight.
We use this chain  to repair missing chain or addind a second chain for safety if required.
Many AMI J Jukeboxes did not come with a front grille  chain. These fronts are very heavy so you want to add a safety chain.
The grille  hooks into cleats at the bottom but we have seen front grills fall to the floor. One you have your grille in the cleats, hook the chain .
This chain is sold by the foot so order as much as you need and you can cut to length later.
